When you submit an appointment request, we collect the information you provide: your name, phone number, email address if you share it, the general reason for your visit, and your insurance carrier if you share it. We also collect standard technical information such as the page you submitted from.

How we use it

We use this information for one purpose: to contact you about scheduling your appointment and verifying your insurance coverage. We do not sell your information, and we do not use the health-related details you share for advertising.

Advertising measurement

These pages may use Google advertising measurement to record that an appointment request happened. This measurement does not include the reason for your visit or any health information you enter in the form.

Your choices

You can request access to or deletion of the contact information you submitted by calling us at (718) 648-0888 or emailing info@ptofthecity.com. If you become a patient, your medical records are governed by our clinic privacy practices, which you will receive at your first visit.
